工作介绍
Experience as a Case Manager, knowledge of Home Care Packages including rostering and budgeting. Relevant tertiary qualifications in behavioural sciences, nursing, allied health, or social work, intermediate level skills with Microsoft Office, clear and concise written and verbal communication in Mandarin or Cantonese and English, a current drivers licence is essential to attend meetings and appointments. This is ideally suited to someone who may be in a similar role but is feeling a bit stale and would love the chance to join a fantastic, high performing team.岗位要求:
1、Report to the Service Manager for Home Care;
2、Manage a portfolio of clients, their carers and family;
3、Facilitate and provide effective care management of Home Care Packages;
4、Meet prospective Home Care customers to promote the services offered;
5、Manage service implementation;
6、Support the customer service team with rostering, timesheets and budgeting;
7、Ensure all plan documentation is accurate。
